Refer a Family

Guardian House offers several programs to support families. Below are details on who can refer, required documentation, and how to get started.

Supervised Visits

Referral Required: Only an attorney or judge may refer a family to this program.

Required Documentation: A court order is required.

Self-Enroll Option: Families may self-enroll. Visit our Enroll in Services page.

Referral Option: Attorneys, judges, or CPS caseworkers may also refer families to this program.

Step-by-Step Guide for Supervised Visitations and Monitored Kids Exchanges

1) After referral, both parties schedule an intake interview

  • Each party must contact Guardian House directly to schedule their intake interview.
  • Staff will not contact the other party to schedule.

2) Submit all required items

  • Both parties must complete all intake paperwork, submit required documentation, and pay applicable fees.
  • Enrollment is not considered complete until all items are received.

3) Child Orientation (for Supervised Visitations)

  • After enrollment is complete, each child participating in supervised visits will attend a Child Orientation before visits can begin.

4) Notification of readiness

  • Once orientations are complete, Guardian House will contact both parties by phone to inform them that the non-custodial party may begin requesting visits.

5) File retention

  • If services do not begin, intake paperwork remains on file for six months.
  • After six months, a new intake interview and intake fee will be required to reactivate or begin services.
